rename "get_hotpotatoes_xxx" functions to "hotpot_get_xxx";
rename "seed_hotpotatoes_RNG" function to "hotpot_seed_RNG";
initialize all $question objects using $this->defaultquestion (if possible);
fix decoding of ampersands in HP quiz readings
