Context: Sprint 3 design for May 1st demo. Hybrid
TTS (live Piper + MP3 fallback), NPC state machine,
QR scanning, difficulty adaptation.

Changes:
- Full design spec with architecture, components,
  data flow, testing strategy, and risk assessment

Impact: Unblocks Sprint 3 implementation planning.
