Revise Copyright statement to align with TLF
...
Recommendation is to avoid using the year nomenclature as this
information is already encoded in the git repo. Avoids needing to
repeatly update.
Also updates AUTHORS.txt from current repo with contributor names

Fix 3D Coverity issue, other minor fixes

* CID 143740: Null pointer dereferences (NULL_RETURNS)
A few minor cut/paste and style issues in the 3D code are also fixed.
