Modified the grades visualisation to make use

of styles a bit better. Works fine under standard
and wood personalise it aparently without problems.
Sure it can be improved...!

Fixed links to help files. Help files were also added under /lang/en/help/grade to remain consistent with the site location of the gradebook as 'grade'

OK, lots and lots of cleanups here. Too many to list, but lots of
...
logic fixes, API fixes, variable fixes etc ... there is still plenty
to do but this is enough to check in and get others working on it.
