Fixed import groups page layout and navigation
...
While working on MDL-26252, I realized that group/import.php page had
issues with layout. Side blocks were missing and the breadcrumb navigation
broken (leading to a non-existing script and having the link to the
current course duplicated).
This patch sets the standard layout for the page so that block are
displayed, fixes the page position in the navigation tree and the
breadcrumb navigation.

MDL-22033 groupings now enabled unconditionally, groupmembersonly kept as experimental - this simplifies groups code logic a bit, user interface changes should not be significant because the groupings are not visible much until at least one created; hopefully the new description of groupmembersonly will have to preventsome gradebook complaints

MDL-21652 html_table rendering refactored
...
* class html_component does not exist any more
* class html_table rendered via html_writer::table()
* html_table, html_table_row and html_table_cell have public $attributes property to set their CSS classes
* dropped rotateheaders feature, should be added again after more research of possible ways (<svg> is not nice IMHO)
* dropped possibility to define CSS classes for table heading, body and footer - can be easily done and better done using just table class and context
