Tim Hunt 68d369917e MDL-70556 core db: change course fullname to 1333 chars 
This is mainly for the benefit of multilang users.

As well as changing the database schema, it is also necessary to change
where this is inforced in PHP code (forms, web services). This was missed
when shortname was changed from 100 to 255 chars some time ago, but I have
fixed that here.

Note, I have not changed the legacy mnet code, since as far as I am aware
that is deprecated.

Paul Holden 0645319e62 MDL-72309 course: make category a required field in forms. 
Course category fields were changed en masse from simple select
elements to autocompletes in 93d72205, but weren't defined as
required fields. This could lead to exceptions and/or unexpected
behaviour if their values were cleared prior to form submission.

David Mudrak 29cbe43189 MDL-13481 Strings fullname and shortname are now deprecated 
This is a final cleanup commit of fullname and shortname issue. All
places where these strings were detected yet have been replaced with
proper fullnamecourse or fullnameuser or some other context specific
string.

tjhunt e452210a18 course request: MDL-17575 and MDL-6160 further improvements: 
1/ email notification when course requested
2/ course request capability so that we can limit who can request new course
3/ list of my pending course requests on the request form

Implemented in 1.9 by Petr Skoda, and reviewed and merged to HEAD by me.

tjhunt 77eddcd527 course request: MDL-9854 major cleanup to the course request code. 
* Rewrite all the forms to use formslib and improve usability.
* Put the place where admins need to go to process approvals in the admin menu.
* Show the request course/pending requests button on the page for the category that requested courses get added to.
